NOTE:
- As of 2011, the US Navy has removed a soldier’s SSN (Social Security Number) from all forms and ID cards but does not address dog tags.
- We have only seen SSN used on line 3.
- If you would like to use your DOD on the dog tags, you can make the number fit by listing the blood type on that line without a space, as: APOS.
- If you do not want your DOD number or SSN, put your phone number, date of birth, other number, or leave it blank.
- A comma after the last name is common if both last and first name are on Line 1. If the names are on two lines, no comma is used.
- A period after the middle initial is optional, may also depend on available space.
- You do not have to follow this format. You can put any text on any line.
Rolled Edge to Front (the rolled edge can be seen when reading the text) is how we usually stamp Current Issue dog tags.
WWII and Korea era dog tags were stamped with the rolled edge to the back (the rolled edge can not be seen when reading the text).
