• As of May 2016, the US Government uses a soldier’s SSN (Social Security Number) on Air Force dog tags.
• If the last and first name does not fit on Line 1, it’s too long, then stamp last name on Line 1 and first name middle initial on Line 2. Thus Line 3 will have SSN, Line 4 the blood type, and Line 5 the religious preference.
• If you do not want to put your SSN, put your phone number, date of birth, other number, or leave it blank.
• If you would like dashes, within the numbers of your SSN (common), put them in the text boxes. We do not automatically add any characters to the inputted text.
• A comma after the last name is common if both last and first name are on Line 1. If the names are on two lines, no comma is used.
• A period after the middle initial is optional, may also depend on available space.
• You do not have to follow this format. You can put any text on any line.
